How does Perosa Topical Cream work?

Perosa Topical Cream acts by paralyzing the mites, a scabies-causing organism, by damaging their nerve cell membrane. Thus, leading to the death of mites and the spread of disease.

Side Effects of Perosa Topical Cream

In some individuals being treated for scabies, skin discomfort such as burning, stinging, or tingling may occur shortly after applying the cream. Other temporary irritation symptoms may include redness (erythema), swelling (edema), eczema, rash, or itching (pruritus).

• Avoid alcohol as it may increase inflammation and worsen the condition.
• Avoid sharing personal belongings such as combs, towels, scarves, and razors.
• Wash the bedding and clothes with soap and hot water after every use.
• Avoid scratching the skin as it may worsen the infection.
• Avoid direct skin-to-skin contact. Seek medical help if your sexual partner also develops scabies.

Indication of Perosa Topical Cream

Perosa 5% cream is used for the treatment of scabies.

Pharmacology

Permethrin cream contains Permethrin, a pyrethroid compound used as a topical scabicidal agent for treating infestations caused by Sarcoptes scabiei (scabies). It disrupts the sodium channel function on the nerve cell membrane of the mites, causing delayed repolarization, which results in paralysis and the eventual death of the pests.

Permethrin is applied topically, and there is limited data on its systemic absorption. A small fraction (less than 0.5%) of the cream is absorbed through the skin within the first 48 hours. Any absorbed permethrin undergoes rapid metabolism via ester hydrolysis, likely in the liver, and its byproducts are excreted primarily through the urine.

Dosage of Perosa Topical Cream

• Adults and children over 12 years: Apply a full tube.
• Children aged 6-12 years: Use up to half a tube.
• Children aged 1-5 years: Apply up to one-quarter of a tube.
• Children aged 2 months to 1 year: Use up to one-eighth of a tube.
• For children under 2 months, the appropriate dose is not established.

It is important to follow a doctor’s prescription when using the cream.

Administration of Perosa Topical Cream

Before applying permethrin cream, ensure the skin is clean, cool, and dry. Avoid hot showers or baths just before use.

For adults and children over 2 years: The cream should be applied to the entire body from the neck down. It should be gently rubbed into the skin until fully absorbed, covering all areas, including between fingers and toes, under the nails, and on the soles of the feet.

For babies under 2 years: Apply to the face, neck, ears, and scalp, avoiding the areas around the eyes and mouth.

Leave the cream on for at least 8 hours before washing it off. If any area of the skin is washed during this time (e.g., after handwashing), reapply the cream to that area. If required, the treatment can be repeated after 7 days based on a doctor’s advice.

Interaction

Treatment of eczema-like reactions with corticosteroids should be discontinued before using permethrin, as corticosteroids may suppress the immune response to the scabies mites, potentially worsening the infestation.

Contraindications

Permethrin should not be used by individuals with known hypersensitivity to any components of pyrethroids or permethrin. Healthcare personnel who regularly apply permethrin may wear gloves to prevent hand irritation.

Pregnancy & Lactation

Due to the lack of specific studies in pregnant women, permethrin should only be used during pregnancy under medical supervision. It is not confirmed whether permethrin is excreted in breast milk, so nursing mothers are advised not to use the cream unless recommended by a doctor.

Precautions & Warnings

Scabies infestations are often accompanied by itching, swelling, and redness. These symptoms may temporarily worsen following treatment with permethrin. Patients should take care to avoid contact with their eyes during application, and if the cream does get into the eyes, they should immediately rinse with water.

Store the cream in a cool, dry place, away from light. Ensure it is kept out of the reach of children.
